The sinking of the tug was due to inadequate understanding of weather conditions, disregard to IMD weather advisories, inadequate communication, and inappropriate manning.
- Lessons Learnt:
- Consider weather forecast and vessel's operating envelope.
- SPM operator and port authority should work closely, prioritizing safety.
- Monitor small vessels closely.
- Manning should meet minimum safe manning levels.
