Identification Marks: To be filled
Father's Name: To be filled
Date of Birth: To be filled in (Day) (Month) (Year) format
Permanent Address: To be filled in block capitals
Nomination: Seafarers are to nominate a person to receive the amount standing to their credit in the event of their death
Certification: The declaration is to be certified by the seafarer's employer or an authorized officer

Seafarers can modify their nomination by submitting a written notice to the Seamen's Provident Fund Commissioner.

Cancellation of Previous Nomination: Seafarers can cancel their previous nomination by submitting a written notice
New Nomination: Seafarers can nominate a new person to receive the amount standing to their credit in the event of their death
Certification: The modification notice is to be certified by the seafarer's employer or an authorized officer
