Key Changes and Requirements

1. Revised Application Forms and Guidelines

Revised application forms (Form A & B) and guidelines for fresh CDC issuance and renewal/replacement have been introduced. These are effective immediately.

Forms: Revised Form A & B (Annexure I & II)

2. Online Registration and Profile Update

All seafarers with INDOS numbers must compulsorily register on the DGS website and update their details within one month. This includes training courses, COC courses, and sea experience.

Action: Register and update profile on DGS website

II/4 Revised Forms and Guidelines

Non-compliance may result in delayed CDC issuance or renewal.
