Key Changes and Requirements
1. New Application Form (Form 15 (EXN-3))
The application form has been revised and is now exclusively used for MEO 1998 examinations. It is serially numbered and should not be photocopied or printed at MMDs.
- Usage: Issued individually to candidates; one form per grade of examination
- Transfer: Sent to the candidate's chosen center if appearing elsewhere; center of eligibility fills Part III and sends it to the Directorate
2. Additional Forms (Form DO and Form IN)
These forms are used to inform candidates of deficiencies in their submission (Form DO) or ineligibility for the examination (Form IN).
- Availability: Copies have been distributed; make more if needed
