Key Changes and Requirements
1. Updated ISM Forms
Amended ISM forms are now annexed to this circular. They include:
- ISM-01, ISM-02, ISM-03, ISM-11, ISM-12, ISM-13, NCR-OBS, AUDIT LOG, ASSESSMENT REPORT (SHIP), ASSESSMENT REPORT (COMPANY), DOCUMENT REVIEW, AUDIT SCHEDULE, ASSESSMENT CHECKLIST, MONITORING REPORT
2. New Form Format and Usage
The new forms are more user-friendly and come with a 'Read Me' reference document. They should be used for transmitting information to the Directorate from 1st April, 2009.
