Instructions for Letter of Authority DGS file number where the approval of Proforma B of the candidate

1. Letter of Authority Details

Update Letter of Authority (LOA) with the following details:

• Serial number: EXN 45 booklet serial number
• INDOS Number: LBS College issued number
• Port of issue: Port where LOA is issued
• DGS reference: File number where Proforma B approval is mentioned
• Special conditions: Reference number if candidate appeared under specific conditions
• Passing date: Date candidate completely passed the exam
• Candidate's name: Full name of the candidate
• Grade of exam: Grade for which candidate passed
• Success date: Date candidate declared successful
• Signature: Examiner of Engineers' signature
• Concurrent certificates: Number and issue date of any concurrent certificates
• Revalidation details: Service or category under which revalidation sought
• Rotation number: Candidate's rotation number (if applicable)
• Physical marks: Brief description of any physical marks
• Photo & verification: Affix stamp-sized color photo with 'Verified' rubber stamp and examiner's initial
• Issue date: Date LOA is issued to the candidate
• ID details: CDC number and place of issue, or passport number and date of issue
