Effective Date:
04.04.2013
Superseded Circulars:
None mentioned
Key Changes and Requirements
1. Incident Summary
An Indian flagged oil tanker, while berthing at Haldia, sustained hull damage resulting in minor oil pollution.
2. Cause of Incident
The vessel's bow moved faster than the stern towards the jetty, and contact with an unprotected fender resulted in hull breach and oil leakage.
- Key factors: Fast rate of turn, lack of tug 'pull' capability, inadequate fender protection
3. Lessons Learned
- Pilotage: Use only very experienced and trained pilots for such vessels at HOJ 3, Haldia
- Tug operations: Ensure tugs can perform both 'push' and 'pull' operations and are made fast to the vessel as required
- Bridge team coordination: Conduct proper Pilot-Master exchange of information before pilotage operations
Important Note
Masters should ensure that fenders are adequately protected to prevent such incidents.
